COPY OF YOUR CHILD'S IMMUNIZATION RECORD
Immunization records expire after one year. Board of Health madates that we have up-to-date immunization records for each camper.**
**NO CHILD WILL BE ALLOWED TO PARTICIPATE IN CAMP UNTIL A SIGNED WAIVER,
HEALTH CARE FORM AND IMMUNIZATION RECORD FROM YOUR CHILD’S DOCTOR
ARE RECEIVED BEFORE THE FIRST DAY OF CAMP.
NO EXCEPTIONS.
WETSUIT ORDER FORM
We have had several requests for the option to purchase a wetsuit for your camper on sign-up day. Though it's not required, it is recommended. After sign-up day, all wetsuits will be ordered and may be picked up on your child's first day of camp!
